You will be asked to fill out a brief medical questionnaire before your surgery. This may be completed online through your patient portal or in person on the day of your procedure. Please allow extra time for check-in if completing forms onsite.

🩻 Imaging & Records

We require up-to-date imaging (X-rays, MRI, or CT) as part of your surgical planning. If you have studies from an outside facility, please bring:

  • A CD or printed copy of your images
  • The radiologist’s report
  • Any operative reports from previous surgeries

If your surgery involves the lower body (hip, knee, or ankle), we recommend bringing a pair of shorts for your exam. Disposable shorts will also be available at the clinic if needed.
